Ishiyama was drafted by the Kintetsu Buffaloes in the 5th round of the 1969 NPB draft, and he never had more than 20 at-bats a season before the Buffaloes released him in 1983. On his last at-bat against Koji Mori, Ishiyama crushed a solo shot. He used 14 seasons to hit his first career home run, and he set the NPB record. Ishiyama then signed with the Samsung Lions of the KBO, and he hit .277/.315/.372 in 64 games in 1984. He slumped to .208/.286/.308 in 1985, and he struggled again as his batting line was .155/.193/.190 in 40 games in 1986. Ishiyama announced his retirement after that season, and he was the bullpen coach of the Buffaloes from 1993 to 2004. He then managed the minors team of the Doosan Bears in 2013, and he became their top team manager in 2014. Ishiyama was the first Japanese manager in KBO, and he was the second foreign manager in KBO history, following Jerry Royster. His Bears was 59-68, and he was fired. Ishiyama then scouted for the Rakuten Golden Eagles.
Overall, Ishiyama hit .222/.276/.305 with 78 hits in 3 seasons in the KBO.
