Organized Baseball suspends a player 50 games for the first positive steroid test; this previously was 10 games. A second positive causes a 100-day suspension and a third positive a lifetime ban.

10 major leaguers were suspended during the 2005 season, the first season in which MLB instituted penalties for positive steroid tests. Nippon Pro Baseball enacted a steroid policy in 2007. Rick Guttormson was the first player to be suspended under Japan's new steroid rule.

This is a partial list of players who have tested positive for steroids since such testing began.
